How to Dial Japan from the US
The format for calling Japan from the US is:
Exit code (011) + Country code (81) + Area code + Local number
For example, to call the Tokyo landline 03-1234-5678: 011-81-3-1234-5678
To call the Japanese mobile 090-1234-5678: 011-81-90-1234-5678

Important: drop the leading 0 from Japanese numbers. Tokyo is 03 locally, but you dial 3 when calling from abroad.
Japan Landline vs Mobile Rates: Why the Difference?
